Summary /Objective

As an LVN, you will contribute to the patient experience from start to finish. This role demands multi-tasking in a fast-paced environment. Your role will be to provide nursing services, training, and support to address the medical and/or psychiatric needs of patients with brain injuries, intellectual and developmental disabilities, substance use disorder, and/or has an emotional or behavioral disability.

Our ideal candidate has solid experience as a Licensed Vocational Nurse and can handle stressful situations. This position requires team participation skills and a problem-solving attitude.

 

Essential Functions

 

·      Taking and monitoring vital signs and the health conditions of patients, ensuring quality services are provided at all times.

·      Dispense and/or administer medication as prescribed by the physician and provide detailed documentation.

·      Oversee and monitor team members who dispense/administer medications.

·      Provide medical treatment prescribed by physician’s orders and personal care services.

·      Conduct patient assessments and implement healthcare services included in each Individual Service Plan (ISP).

·      Respond to changes of condition.

·      Patient education and training to include prescribed treatments, smoking cessation, sex education, STD prevention, and self-medication.

·      Serve as an advocate and liaison with other medical and ancillary service providers including coordination of services to ensure consistent treatment with external agencies.

·      Provide staff training / in-services including universal precautions, medication administration, and infection control.

 

  • Moving patients safely.
  • Observing patient reactions to medications.
  • Observing patient mental health status.
  • Providing emotional support.

Qualifications

·      Valid CA LVN license

·      Associate degree in Nursing or related field, or state certificate

·      One year of experience working with population served in an LVN capacity

·      In-depth knowledge of rehabilitation facility procedures

·      Thorough knowledge of the best practices in healthcare

·      Great people skills with the ability to effectively communicate with staff, patients, and families

·      CPR certification

Education/Experience/Skills:

  • Completion of an accredited Licensed Vocational Nursing training program.
  • Demonstrates professional competence and clinical reasoning in assessing patient’s condition at the onset and through all phases of care.

·      Candidate must have strong knowledge of OSHA regulations, ensure HIPPA compliance. Understanding and always ensure the concepts of universal precautions and OSHA certified.

Work Environment

This job operates in a clinical and office setting. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. 

 

Physical Demands

The ability to stand and sit for hours at a time, (with some bending and stooping), ability to use manual dexterity in relation to clinical requirements, and ability to lift 50 lbs.
